The Physical Therapist Essential Duties Are:

We provide superior care for patients in families in homes and facilities. The PT’s assure timely provision of therapy services pursuant to the individualized care plan.

Qualifications:

  • Graduate of an accredited physical therapist education program
  • Must remain current on mandatory PT in-service requirements and provide documentation.
  • Must be licensed in the State of Ohio as a physical therapist.
  • Prior home health experience is desired.
  • Demonstrated ability to work independent of supervision.
  • Must demonstrate at least 1 year prior medical-surgical PT experience.
  • Must be able to perform duties including frequent standing, walking, sitting and occasional stooping, kneeling, reaching/working overhead, stair climbing, pushing, pulling, and lifting/carrying.
  • Ability to drive during daytime, nighttime or inclement weather.
  • Valid Driver’s License with Safe Driving Record
  • State Minimum Automobile Insurance Coverage
