 WIOA Youth Program Eligibility Application (JFS 03002)  WIOA Youth Program Eligibility Application (JFS 03002)  Proof of residency (home utility bill, current lease, mortgage, deed, official mail from a Federal, State, or City agency) Public Assistance ID Number
 Identification: School ID, State ID/License
 Social Security card (copy is acceptable)
 Birth Certificate (copy is acceptable) Public Assistance ID Number  CCMEP Family Income Calculation form

The following documents are acceptable as proof of income:

  • Two (2) consecutive pay stubs dated within the last 30 days (must include payee name and gross income)
  • Current Pension Award Letter
  • Unemployment Benefit Document(s) dated within the last 30 days
  • If self-employed, a 30-day calendar with name of business, the service provided, and the amount of funds received

Please note:

  • All Interns ages 14-15 MUST have a work permit for the Summer Session, and all Interns must have a work permit for the Fall Session and Winter Session
  • All males 18 years of age must register with Selective Services
