Salary Range: $55,000 – $90,800
Sign-On Bonus: $2,500
Position Summary
The Licensed Social Worker (LSW/LPC) provides high-quality behavioral health services to clients across Ohio, including children, adolescents, adults, couples, and families. Services include community linkage, skills building, problem solving, and social support. Services may be delivered in office settings, client homes, or community-based locations.
Essential Duties and Responsibilities
- Provide evidence-based mental health counseling to individuals, couples, and families
- Deliver therapeutic services to children, adolescents, adults, and older adults in accordance with best practices and developmental needs
- Conduct comprehensive biopsychosocial and clinical assessments
- Develop, implement, and evaluate individualized treatment plans
- Perform risk assessments and provide crisis intervention services as needed
- Utilize electronic health records (EHR) systems for documentation and care coordination
- Complete accurate and timely clinical documentation in compliance with organizational, state, and regulatory standards
- Connect clients with appropriate community resources and provide ongoing skills building, problem solving, and social support
- Collaborate effectively within a multidisciplinary, diverse care team
- Demonstrate strong communication, interpersonal, and problem-solving skills
- Maintain professionalism and adaptability when working with diverse populations
- Learn, follow, and uphold Carex standards, policies, and best practices
Qualifications
- Current Ohio licensure as Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC)
- Bachelor’s degree in Social Work or related field (Master’s preferred)
- Minimum of 1–2 years of relevant experience
- Experience working with children and adolescents in behavioral health or social services settings
- Experience providing evidence-based mental health interventions
- Experience using EHR systems preferred
Working Conditions
- Combination of office-based work and community fieldwork
- Services provided in client homes, schools, hospitals, and community locations
- Standard schedule: Monday–Friday, with flexibility for occasional evenings or weekends
- Local travel approximately 10–15%; reliable transportation and valid driver’s license required
- May work in high-stress environments and with individuals experiencing crisis or trauma
Physical Requirements
- Ability to sit for extended periods while completing documentation and attending meetings
- Ability to move through varied environments (stairs, uneven surfaces, confined spaces)
- Ability to operate standard office equipment
- Ability to communicate clearly in person, by phone, and in writing
- Ability to lift or carry up to 25 pounds occasionally
- Visual acuity sufficient to read electronic and printed records
- Ability to remain calm and exercise sound judgment in emotionally intense situations
