Position Summary
Elite Care Consultants provides personalized in-home care services, including medication reminders, 24/7 personal care, memory care, transportation, companionship, respite care, and light household support.
The Home Care Caregiver is a vital member of our team, responsible for delivering non-medical, hands-on assistance and companionship to elderly clients, individuals with disabilities, and those recovering from medical procedures. Caregivers work in clients’ private residences to ensure their physical safety, emotional well-being, and adherence to customized care plans.
Key Duties and Responsibilities
The Home Care Caregiver will perform duties including, but not limited to:
- Personal Care Assistance
- Assist with activities of daily living (ADLs): bathing, grooming, dressing, toileting, hygiene
- Provide 24/7 personal care support as scheduled
- Medication Management
- Provide medication reminders
- Monitor and report observable side effects or health changes
- Memory Care Support
- Engage and supervise clients with dementia, Alzheimer’s, or other cognitive conditions
- Ensure safety and emotional stability
- Nutritional Support
- Plan and prepare nutritious meals based on dietary needs
- Assist with feeding when necessary
- Companionship & Emotional Support
- Engage in conversation, reading, games, and social activities
- Promote mental stimulation and reduce isolation
- Light Housekeeping
- Maintain a clean and safe environment
- Perform light cleaning, laundry, and organization of immediate living areas
- Transportation & Errands
- Safely transport clients to appointments and outings
- Assist with grocery shopping and general errands
- Respite & Mother’s Care
- Provide temporary relief for primary caregivers
- Support new mothers with in-home assistance
- Customized Care Plan Compliance
- Follow individualized care plans
- Communicate changes or concerns promptly to the Care Manager
- Documentation & Reporting
- Maintain accurate daily logs
- Report health or environmental concerns immediately
Qualifications & Requirements
Education
- High School Diploma or GED required
Certifications
- CNA or HHA certification preferred (not mandatory depending on state regulations and client needs)
Experience
- Prior experience in home health care, elderly care, or personal caregiving strongly preferred
Background Requirements
- Must pass criminal background check
- Drug screening required
- Reference verification required
Physical Requirements
- Ability to lift, push, or pull up to 50 lbs
- Ability to stand for extended periods
- Ability to assist with transfers and ADLs
Transportation
- Valid driver’s license
- Reliable vehicle
- Active auto insurance (required for transportation duties)

Working Conditions
